Kerala nurses and healthcare professionals who want to work in the UK as a Registered Nurse — either already in Kerala, currently working in the Gulf, or elsewhere in India. The process is the same regardless of where you currently are.
The Big Picture First
To work as a Registered Nurse in the UK, you must be registered with the Nursing and Midwifery Council (NMC). This is the UK's nursing regulatory body — without an NMC PIN number, you cannot practise as a nurse anywhere in the UK.
The NMC registration process for overseas nurses has five broad stages:
- English Language Test — IELTS Academic or OET
- NMC Online Application and Document Submission
- CBT — Computer Based Test (theory exam, done in India)
- OSCE — Objective Structured Clinical Examination (practical exam, done in the UK)
- Receive NMC PIN — you are now a registered nurse in the UK
Total typical timeline from starting English preparation to receiving your NMC PIN: 12 to 18 months. It can be done faster — but budget for this realistically.
Stage 1 — English Language Test
You have two options — IELTS Academic or OET (Occupational English Test). Both are accepted by the NMC. Many Kerala nurses prefer OET because it is healthcare-focused, but IELTS is more widely available in Kerala.

Many nurses underestimate the Writing section. This is where most candidates fall short. Structured preparation is critical — especially for the OET healthcare scenario writing. Do not attempt this exam without dedicated preparation. A failed attempt wastes both time and money.
Stage 2 — NMC Application and Documents
Once you have your English test result, you create an NMC account online at nmc.org.uk and begin your eligibility application. You will need to pay the NMC qualification evaluation fee of £140 (approximately Rs.15,000) at this stage. This fee is non-refundable even if you are found ineligible.
Documents Required
- Valid passport
- Nursing qualification certificate and transcripts
- Good Standing Certificate from your nursing council (Kerala Nursing Council, or DHA/SCHS if from Gulf)
- Employer reference letter
- Good Character declaration from a colleague
- Police Clearance Certificate (PCC)
- Good Health Certificate from your doctor
- English language test result certificate
Getting your Good Standing Certificate from Kerala Nursing Council can take time. Apply for it early — do not wait until you need it. The NMC requires it to be current and valid.
The NMC aims to process applications within 60 days of receiving all documents. In practice, allow 3 months to be safe. Once approved, you will receive your Pearson VUE account details to book the CBT.
Stage 3 — CBT (Computer Based Test)
The CBT is a multiple-choice theory exam testing your core nursing knowledge and clinical decision-making. The major advantage is that you can take the CBT in India — at a Pearson VUE test centre. There are Pearson VUE centres in major Kerala cities including Thiruvananthapuram, Kochi, and Kozhikode.
CBT Structure
- Part A — Numeracy (medication calculations)
- Part B — Clinical knowledge (multiple choice questions)
CBT fee: approximately £130 (around Rs.14,000). The CBT result is valid for two years. Once you pass the CBT, you can start applying for UK jobs — NHS hospitals will often provide sponsorship and support to bring you to the UK for the OSCE.
Stage 4 — OSCE (Objective Structured Clinical Examination)
The OSCE is the practical skills examination and it must be taken in the UK. This is the most significant step — and the most challenging. There are five approved OSCE test centres across the UK.

The OSCE consists of 10 stations including:
- Four skills-based practical stations (medication administration, wound care, vital signs)
- Four APIE stations (Assessment, Planning, Implementation, Evaluation of patient care)
- Two silent skills stations
- NHS constitutional values and the 6Cs of nursing assessed throughout
- OSCE fee: £794 (approximately Rs.85,000)
- You have a maximum of three attempts
- Must wait at least 10 days between attempts
- Resit fee (7 or fewer stations): £397
- Many NHS trusts reimburse the OSCE fee — confirm this in your offer letter
- You can work in a supervised pre-registration role while awaiting your first OSCE attempt
Preparing for OSCE in Kerala
ASAP Kerala (Additional Skill Acquisition Programme) runs a Certificate of OSCE Proficiency programme in partnership with Focus Active Learning. Stage 5 — Receive NMC PIN
After passing both CBT and OSCE, you return to your NMC portal and complete the final stage of your application. The NMC reviews all documents and test results and, if satisfied, issues your NMC PIN — your official registration number that allows you to practise as a Registered Nurse anywhere in the UK.
